Without a ruler for reference, it is hard to determine that this is a hypertrophied prostate. It is important remember that benign prostatic hypertrophy (BPH) is present in about 20% of males at age 40, 70% at age 60 and 90% at age 80 and is associated with considerable morbidity. The underlying process is hyperplasia of the cells of the organ and not hypertrophy of the individual cells. A relationship between BPH and prostatic carcinoma has not been established.
